1. Stop by for a Tour

If you are looking to enroll your loved one at Page Robbins, stop by for an informal tour to see our Center and ask any questions. We recommend that just family members, and not the potential client, come for the initial tour. Your loved one whom you are wanting to enroll can see the Center during the assessment. We are open Monday to Friday, 6 a.m. to 6 p.m. You may drop by any time between 8 a.m. and 4 p.m. to take a tour. If you would like to take a tour before 8 or after 4, please call (901-854-1200) to let us know in advance what time you will be stopping by.

2. Fill Out An Application and Other Required Paper Work

You may download and print these forms or pick them up while on your tour. Concerning the Physician’s Form, a visit to see your loved one’s physician may not be necessary. You may be able to send and have the form completed and returned via fax or email, depending on the physician office’s preference and the length of time that has passed since your loved one’s last appointment. Any physician who has good knowledge of your loved one’s past and current medical history can complete the form.

3. Schedule an Assessment

After you have toured and completed an application, it is time to schedule an assessment. Please note that paperwork must be complete for an assessment to be scheduled. Assessments last around 2 hours and are scheduled with Sheri Wammack, our Program Director. During this meeting, the primary caregiver will meet with Sheri to discuss the admission paperwork and level of care, and the potential client will participate in an activity and have lunch. After the assessment, your loved one can start as soon as the next day (depending on the days of the week he or she is scheduled to attend).
