While we would love to say “the more, the merrier,” we have a new, lower capacity for the number of clients we can schedule each day. When we reach that capacity, a waiting list will begin for those days.

Getting Started

1. Set up a Virtual Tour

The Center is currently closed to outside visitors in order to keep our vulnerable population safe in this pandemic.  If you are looking to enroll your loved one at Page Robbins, set up a time to tour Page Robbins virturally.  Call or email our Program Director, Sheri Wammack at sheri@pagerobbins.org or 901-854-1200.

2. Fill Out An Application and Other Required Paper Work

You may download and print these forms or pick them up at the Center (just park under the portico and call us when you get here, and we’ll bring the forms out to your car).

For the Physician’s Form, a visit to your loved one’s doctor may not be necessary. You may be able to have the form completed via fax or email, depending on the physician office’s preference and the length of time that has passed since your loved one’s last appointment. Any physician who has good knowledge of your loved one’s medical history can complete the form.

These forms will need to be turned in before the assessment can be scheduled. Please note that we are requiring all new clients to provide proof of a negative COVID-19 test prior to the assessment.

3. Schedule an Assessment

After you have toured and completed an application, it is time to schedule an assessment. Paperwork must be complete for an assessment to be scheduled.

Assessments last around 2 hours and are scheduled with Sheri Wammack, our Program Director. During this meeting, the primary caregiver will meet with Sheri to discuss the admission paperwork and level of care, and the potential client will participate in an activity and have lunch.

After the assessment, your loved one can start as soon as the next day (depending on the days of the week they are scheduled to attend).
